I searched for information in my mind and managed to discover the reason behind it. Normally, this happens when a sword cannot withstand the strength or magic of an adventurer.
In other words, if a level four or higher adventurer uses a low-quality sword, it will develop this anomaly and break quickly. Once a sword acquires this anomaly, it will shatter in at most four swings.
If the adventurer uses magic, two swings are enough.

Bella's eyes were shining as she stared at her sword. Hephaestus stood up and walked over to us. She took the sword in her hand and said, "You made a lot of changes. It's practically a new sword. At least you didn't change the material used to forge it."

"Hey." Hephaestus elbowed me, and I smiled.
"I'm kidding."
"What was wrong with the sword?" Hephaestus asked Bella, not me.
"I used it to kill two goblins on the second floor of the dungeon, and a crack appeared on the blade. I didn't think that was normal since it's a new sword, so I came here to ask for help."
"I see. That shouldn't have happened. Someone probably made a mistake while forging it. Can you tell me which shop you bought it from? I'll take care of this."
"Don't be hasty, Hephaestus," I said with a sigh.
I looked at Bella again and noticed she was level three. I can't see her Falna, but I know she's level three.
In the original story, Bell—who was a man—wasn't at such a high level at the beginning. It took several episodes for him to reach level two. 'But... I don't know what point in the story I'm in.'
The story may have been completely altered because of my existence. Bell's gender being changed is proof of that.
"What do you mean?" Hephaestus asked.
"That sword wasn't perfect, but it shouldn't have broken so easily. It's probably related to one of Bella's skills or her lack of magical control."
"A skill?" Bella looked confused. "I don't think any of my skills caused that."
"Are you good at controlling your magical power? Are you good at controlling your attacks and skills?"
"Well…" Bella averted her gaze.
"You're not good at it, are you?"
"…"
"You're probably using more magical power than necessary when activating the sword's abilities. That's overloading the blade… and that's what caused the anomaly."
"Are—are you serious?" Bella's eyes widened.
"Yes, I'm serious. Why do you think inexperienced adventurers' weapons break more easily?"
"But that's… how am I supposed to fix that?"
"By training," I said with a laugh. "But don't worry, you can use this sword without any problems. I greatly increased its durability and reinforced it. It probably won't develop any anomalies. You don't need to worry about it breaking in the middle of a battle."
"You can trust him. He's very good at what he does," Hephaestus said, slapping my back.
"Of course I trust God Eryk! Thank you very much!" Bella gave a wide smile and bowed her head respectfully. "But… I don't have any money to pay you." She started to look nervous.
"Don't worry. He's not going to charge you for this, right?" Hephaestus looked at me.
"Who said I won't— Hey! That hurts!"

"Yes, that's right. Don't worry." I smiled at her. "I was just joking. You didn't need to punch me."
"…" Hephaestus smiled, then looked at Bella and said, "You can go now. Ah, and tell Hestia to come visit me."
"Alright, thank you very much!" Bella bowed several times.
When she left the room and closed the door, I turned to Hephaestus and said, "That girl is very special." Knowing Bella's past and how strong she would become, I couldn't say anything else, could I?
"So you really like her. Bastard," Hephaestus said, pretending to be irritated.
